Filing and Maintenance Requirements

1. Sebuah perusahaan melakukan kegiatan usaha komersial, harus mendapatkan SIUP 2. SIUP, sebagaimana dimaksud pada ayat (1), terdiri dari:   Sebuah. SIUP Kecil (dikeluarkan untuk usaha komersial skala kecil); b. SIUP Menengah (dikeluarkan untuk usaha komersial skala menengah); c. SIUP Besar (dikeluarkan untuk usaha komersial skala besar) Bupati atau Walikota berwenang untuk daerah yang dikeluarkan SIUP.For perdagangan bebas dan pelabuhan bebas, bupati atau walikota didelegasikan kewenangannya, diatur dalam Pasal 3, untuk perdagangan lokal gratis Area Badan Penyelenggara gratis pelabuhan.- 1. 1. permohonan SIUP Kecil, Menengah, atau Besar oleh perusahaan harus dilengkapi dengan Bupati atau Walikota, dalam hal ini, kantor pelayanan sipil yang bertanggung jawab untuk kegiatan komersial lokal, dengan menyelesaikan Model A SP-SIUP Kecil / Menengah / Besar , sebagaimana tercantum dalam Lampiran Keputusan ini. 2. Aplikasi, sebagaimana dimaksud pada ayat (1), harus ditandatangani oleh pemilik / Direktur / wakil company.1 tersebut. Aplikasi, yang ditentukan dalam Pasal 11, harus dilengkapi dengan dokumen sebagai berikut:   Sebuah. Terbatas Perseroan:   1. salinan akta notaris pendirian: 2. Salinan surat persetujuan yang dikeluarkan oleh Menteri Kehakiman; 3. copy KTP (KTP) pemilik / Direktur / perwakilan; 4. copy NPWP; 5. copy Tempat Usaha Izin (SITU) yang dikeluarkan oleh pemerintah daerah, untuk usaha komersial yang diperlukan untuk memperoleh SITU, berdasarkan Undang-Undang Gangguan 6. Neraca.   b. koperasi:   - Copy akta pendirian, yang telah memperoleh persetujuan dari instansi yang berwenang; - Copy KTP (KTP dari ketua / wakil); - Copy NPWP; - Copy dari tempat Izin Usaha (SITU) yang dikeluarkan oleh pemerintah daerah, untuk usaha komersial yang diperlukan untuk memperoleh SITU, berdasarkan Undang-Undang Gangguan. - Neraca keuangan.   c. Perusahaan, yang tidak terbatas atau koperasi:   1. Kemitraan:   Sebuah. salinan akte pendirian / akta notaris, yang telah terdaftar di pengadilan yang lebih rendah; b. copy KTP (KTP) dari c. Copy NPWP; d. Copy Tempat Izin Usaha (SITU) yang dikeluarkan oleh pemerintah daerah, untuk usaha komersial yang diperlukan untuk memperoleh SITU, berdasarkan Undang-Undang Gangguan. e. Neraca keuangan.   2. kepemilikan Sole:   Sebuah. copy KTP (KTP pemilik / pengelola; b. salinan Tempat Izin Usaha (SITU) yang dikeluarkan oleh pemerintah daerah, untuk usaha komersial yang diperlukan untuk memperoleh SITU, berdasarkan Undang-Undang Gangguan. c. Jika, dalam waktu 15 hari kerja sejak tanggal pengajuan permohonan persetujuan oleh Menteri Kehakiman, penerapan SIUP, sebagaimana dimaksud pada ayat (1) a belum memperoleh surat mengatakan persetujuan, pemohon hanya akan salinan lengkap dari Pendirian Perusahaan data dan fotokopi bukti pembayaran biaya administrasi untuk proses persetujuan tersebut dikeluarkan oleh Departemen Kehakiman, yang merupakan persyaratan untuk memperoleh SIUP. 3. Jika pemohon, sebagaimana dimaksud pada ayat (2), telah memperoleh surat mengatakan persetujuan, ia / dia harus memberikan salinan surat mengatakan persetujuan kepada bupati / walikota, selambat-lambatnya 14 hari kerja sejak tanggal penerbitan surat mengatakan persetujuan kepada bupati / walikota, selambat-lambatnya 14 hari kerja sejak tanggal penerbitan surat kata persetujuan. 4. Companies, which are neither limited liability nor cooperatives : 1. Partnerships : a. copy of deed of establishment/notarized deed, which has been registered with the lower court; b. copy of ID card (KTP) of the c. Copy of NPWP; d. Copy of Business Place Permit (SITU) issued by the regional government, for commercial businesses required to obtain SITU, based on the Disturbance Law. e. Balance sheet. 2. Sole proprietorships : a. copy of ID card (KTP of the owner/organizer; b. copy of Business Place Permit (SITU) issued by the regional government, for commercial businesses required to obtain SITU, based on the Disturbance Law. c. Balance sheet. 2. If, within 15 working days from the date of submission of application for the approval by the Minister of Justice, the application of SIUP, as specified in paragraph (1) a has not obtained the said letter of approval, the applicant shall only complete copy of Company Establishment Data and copy of evidence of payment of administration fee for the said approval process issued by the Department of Justice, which is a requirement for obtaining SIUP. 3. If the applicant, set forth in paragraph (2), has obtained the said letter of approval, he/she shall furnish a copy of the said letter of approval to the regent/mayor, not later than 14 working days from the date of issuance of the said letter of approval to the regent/mayor, not later than 14 working days from the date of issuance of the said letter of approval. 4. If the letter of approval of the applicant specified in paragraph (2) is reject, the application is void. 5.
